Flagler Health, Inc. (“Flagler,” “our,” or “we”) is committed to protecting the privacy and privacy choices of our online visitors and others whose personal data we receive. This Privacy Statement describes our policies and practices on the collection, use, and storage of information on (“Site”), and also applies to personal information which we obtain and process other than through the Site, including personal information about our customers, end-users, vendors, or service providers.

When Flagler accesses patient information, Flagler does so as a Health Insurance Portability and Accountability Act (HIPAA) business associate of a healthcare provider.  Flagler’s collection, use, and disclosure of patient information is governed by HIPAA and the business associate agreement with the healthcare provider, and not by this Privacy Statement.  Consistent with HIPAA and the business associate agreement with the healthcare provider, Flagler uses and discloses patient information only for purposes of the healthcare provider’s treatment, payment, and healthcare operations; and for limited administrative and management purposes of Flagler.

Information we collect on our Site

We have designed our Site so that visitors can navigate and view Site content without providing any personal information to us, other than IP address. This section describes information that visitors might choose to provide, as well as our practices for the automatic collection and use of IP address and other non-personal information and a visitor’s navigation through the Site.

Information you provide. Through your use of the Site, you may choose to provide us with your contact information and name.  For example, a health care professional may fill out a contact form on the Site requesting additional information about our services.

Information we collect automatically. In order to improve the online experience of website visitors, we routinely collect IP address and certain non-identified information about our visitors and their use of the Site. For example, we record the IP address of each computer/device that logs onto our Site, which allows us to maintain communication with your computer/device during your visit. We also use “cookies,” which are small text files that are placed on your computer/device. Cookies allow us to identify how you move through the Site and any specific content you visit, and allow us to identify your computer/device the next time you visit so that we can bring you information you may be interested in. In addition, we hire third parties to provide services for us, and these third parties may also place cookies on your computer. Your Web browser allows you to disable these “cookies,” although parts of the Site may not work efficiently without cookies.

We may also place advertisements on third parties’ websites. These third parties may track website usage information, as well as information on “click-throughs” from an advertisement to our Site. This helps us understand the effectiveness of our advertising. This information is not personally identifiable.

Information provided to us outside our Site

We also receive personal information in ways other than through our website. For example, we collect information about individual representatives of our customers and potential customers for the purposes of communicating with such customers and potential customers and providing our services.  The personal information we collect in this context includes name, contact information, and any other information that is provided to us during the course of doing business.

We also collect information from job applicants in both paper format and digitally.

We also maintain records of our visits with physician customers and their attendance at educational and other events we host.

We also collect information from our vendors, suppliers, consultants, professional advisors and other third parties for the purposes of managing and operating our business.

Personal information is given the same protection by us, whether we receive it through the Site or by other means. This Privacy Statement applies to Personal information collected or received by us, no matter how the information was received.
